Abstract

IgE-mediated food allergy is a transient condition for some children, however there are few indices to predict when and in whom food allergy will resolve. Skin prick test (SPT) and serum-specific IgE levels (sIgE) are usually monitored in the management of food allergy and are used to predict the development of tolerance or persistence of food allergy. The aim of this article is to review the published literature that investigated the predictive value of SPT and sIgE in development of tolerance in children with a previous diagnosis of peanut, egg and milk allergy. A systematic search identified twenty-six studies, of which most reported SPT or sIgE thresholds which predicted persistent or resolved allergy. However, results were inconsistent between studies. Previous research was hampered by several limitations including the absence of gold standard test to diagnose food allergy or tolerance, biased samples in retrospective audits and lack of systematic protocols for triggering re-challenges. There is a need for population-based, prospective studies that use the gold standard oral food challenge (OFC) to diagnose food allergy at baseline and follow-up to develop SPT and sIgE thresholds that predict the course of food allergy.

Σχετικά με την παρακολούθηση της φυσικής πορείας της τροφικής αλλεργίας τα υπάρχοντα δεδομένα δεν φαίνεται να υποστηρίζουν την κοινή παραδοχή πως η παρακολούθηση με  δερματικές δοκιμασίες ή την ειδική IgE επαρκεί από μόνη της για να επιβεβαιώσει ή να αποκλείσει την επιμονή μίας τροφικής αλλεργίας και ο συνεπώς προκειμένου να εκτιμηθούν αυτοί οι ασθενείς θα πρέπει να υποβληθούν σε τροφικές προκλήσεις. Υπάρχει επιτακτική ανάγκη για την διενέργεια προοπτικών κλινικών μελετών στον γενικό πληθυσμό με την χρήση τυποποιημένων τροφικών προκλήσεων τόσο κατά την αρχική διάγνωση της νόσου , όσο και κατά την διάρκεια της παρακολούθησής της προκειμένου να προσδιορισθούν με ακρίβεια οι τιμές των δερματικών δοκιμασιών ή της ειδικής IgE οι οποίες θα αποδίδουν με υψηλή προγνωστική αξία την κλινική ενεργότητα της νόσου.

Abstract

The worldwide prevalence of food allergy appears to be increasing. Early life environmental factors are implicated in the aetiology of this global epidemic. The largest burden of disease is in early childhood, where research efforts aimed at prevention have been focused. Evidence synthesis from good quality systematic reviews is needed. We performed an overview of systematic reviews concerning the prevention and aetiology of food allergy, retrieving 14 systematic reviews, which covered three broad topics: formula (hydrolysed or soy) for the prevention of food allergy or food sensitization; maternal and infant diet and dietary supplements for the prevention of food allergy or food sensitization and hygiene hypothesis-related interventions. Using the AMSTAR criteria for assessment of methodological quality, we found five reviews to be of high quality, seven of medium quality and two of low quality. Overall we found no compelling evidence that any of the interventions that had been systematically reviewed were related to the risk of food allergy. Updating of existing reviews, and production of new systematic reviews, are needed in areas where evidence is emerging for interventions and environmental associations. Furthermore, additional primary studies, with greater numbers of participants and objective food allergy definitions are urgently required.

Οι LodgeCJ και συν σε αυτήν την εποπτική μελέτη , η οποία περιλαμβάνει την ανάλυση 14 συστηματικών ανασκοπήσεων με θέμα την αποτελεσματικότητα  στην πρόληψη της τροφικής αλλεργίας με υδρολυμένα υποκατάστατα μητρικού γάλακτος, προϊόντα με βάση την σόγια, τον χρόνο εισαγωγής στερεών τροφών στην δίαιτα των βρεφών και τα τα συμπληρώματα διαίτης του βρέφους και της μητέρας με προβιοτικά ή ω-3 λιπαρά, δεν εντόπισαν επιστημονικά δεδομένα ικανά να υποστηρίξουν πέραν κάθε αμφιβολίας πως οι πρακτικές αυτές οδηγούν στην μείωση της εμφάνισης τροφικής αλλεργίας.

Abstract

OBJECTIVE:

Food allergies continue to be an increasingly common disorder, however, no treatment strategies are currently approved for the routine management of individuals with food allergies. Encouraging results from early open-label studies have sparked great interest in oral and sublingual immunotherapy, and thus several randomized controlled trials have recently been conducted to establish the safety and efficacy of these treatment strategies. The aim of this review is to examine the recent studies for peanut, milk and egg allergies.

DATA SOURCES:

Open-label and randomized control trials are discussed.

STUDY SELECTIONS:

Studies focusing on peanut, milk and egg allergies are included.

RESULTS:

Current evidence indicates that desensitization is possible for the majority of subjects who undergo oral immunotherapy. Clinical improvement has been associated with favorable immunologic changes, including smaller skin prick test wheal sizes and increased allergen-specific IgG4 levels. Adverse reactions are common, however, and thus safety concerns remain. Sublingual immunotherapy thus far has not proven to be as effective as oral immune-therapy.

CONCLUSION:

Oral and sublingual immunotherapy are promising treatments for food allergy. Optimization and standardization of protocols, along with additional assessments of safety are still needed.

Τα σύγχρονα δεδομένα συνηγορούν πως η από του στόματος απευαισθητοποίηση (ΟΙΤ ή SOTI) στο γάλα και το αυγό όπως και η υπογλώσσια ανοσοθεραπεία (SLIT) στο αράπικο φυστίκι συσχετίζονται τόσο με την κλινική βελτίωση των ασθενών όσο και με την βελτίωση ορισμένων παρακλινικών παραμέτρων (ελλάτωση της δερματικής αντιδραστικότητας στις δερματικές δοκιμασίες δια νυγμού και αύξηση της ειδικής IgG4).Θα πρέπει όμως να σημειωθεί πως οι ανεπιθύμητες ενέργειες είναι συχνές. Αναφορικά με την οδό και την μέθοδο χορήγησης η απευαισθητοποίηση (SOTI) φαίνεται πως υπερτερεί της υπογλώσσιας ανοσοθεραπείας (SLIT), τουλάχιστον στους ασθενείς με αλλεργία στο αράπικο φυστίκι, πιθανόν λόγω των υψηλότερων δόσεων που είναι δυνατόν να χορηγηθούν μέσω SOTI. Επί του παρόντος οι μακροπρόθεσμη αποτελεσματικότητα είτε της απευαισθητοποίησης (SOTI), είτε της υπογλώσσιας ανοσοθεραπείας (SLIT) είναι αβέβαιη.

Abstract

BACKGROUND:

Allergic reactions to food can have serious consequences. This systematic review summarizes evidence about the immediate management of reactions and longer-term approaches to minimize adverse impacts.

METHODS:

Seven bibliographic databases were searched from their inception to September 30, 2012, for systematic reviews, randomized controlled trials, quasi-randomized controlled trials, controlled clinical trials, controlled before-and-after and interrupted time series studies. Experts were consulted for additional studies. There was no language or geographic restrictions. Two reviewers critically appraised the studies using the appropriate tools. Data were not suitable for meta-analysis due to heterogeneity so were narratively synthesized.

RESULTS:

Eighty-four studies were included, but two-thirds were at high risk of potential bias. There was little evidence about acute management for non-life-threatening reactions. H1-antihistamines may be of benefit, but this evidence was in part derived from studies on those with cross-reactive birch pollen allergy. Regarding long-term management, avoiding the allergenic food or substituting an alternative was commonly recommended, but apart from for infants with cow’s milk allergy, there was little high-quality research on this management approach. To reduce symptoms in children with cow’s milk allergy, there was evidence to recommend alternatives such as extensively hydrolyzed formula. Supplements such as probiotics have not proved helpful, but allergen-specific immunotherapy may be disease modifying and therefore warrants further exploration.

CONCLUSIONS:

Food allergy can be debilitating and affects a significant number of people. However, the evidence base about acute and longer-term management is weak and needs to be strengthened as a matter of priority.

Ο σκοπός αυτής της ανασκόπησης είναι η διερεύνηση της αποτελεσματικότητας των σύγχρονων προσεγγίσεων σχετικά: α) με την αντιμετώπιση των ήπιων συστηματικών αντιδράσεων μετά από έκθεση σε τροφικά αλλεργιογόνα β) την μακροπρόθεσμη διαχείριση της ενεργού τροφικής αλλεργίας με αποφυγή της υπεύθυνης τροφής, υποκατάστατα γάλακτος αγελάδας σε ασθενείς με τροφική αλλεργία στο γάλα, προβιοτικά και τεχνικές απευαισθητοποίησης. Ειδικά όσον αφορά το μακροπρόθεσμο όφελος των τεχνικών απευαισθητοποίησης και την επίδραση στην φυσική πορεία της νόσου οι D. deSilva και συν, συμφωνούν με τις παρατηρήσεις των Wang και Sampson (Oral and sublingual immunotherapy for food allergy) :

«It is uncertain whether any gains in tolerance will continue while on treatment or when treatment ceases. Where studies did examine this, tolerance tended to persist only for a few months after immunotherapy ceased.»
