Νεότερες κατευθυντήριες οδηγίες : Οξεία και Χρόνια Ρινοκολπίτις, Ανοσοθεραπεία στα παιδιά

Πρόσφατα δημοσιεύθηκαν τρείς κατευθυντήριες οδηγίες, δύο με θέμα την ρινοκολπίτιδα, οξεία και χρόνια και μία επιπλέον για την ανοσοθεραπεία στα παιδιά .

Από την δικιά μου άποψη ιδιαίτερα σημαντική είναι η αναθεώρηση του 2012 της EP3OS 2007, μίας θεμελιώδους κατευθυντήριας οδηγίας για την αντιμετώπιση του ασθενούς με χρόνια ρινοκολπίτιδα και ειδικά εκείνου του φαινότυπου που χαρακτηρίζεται από την παρουσία ρινικών πολυπόδων και χρόνια ηωσινοφιλική φλεγμονή των κατώτερων αεροφόρων οδών, με ή χωρίς υπερευαισθησία στα NSAID.

The European Position Paper on Rhinosinusitis and Nasal Polyps 2012 is the update of similar evidence based position papers published in 2005 and 2007.

The document contains chapters on definitions and classification, we now also proposed definitions for difficult to treat rhinosinusitis, control of disease and better definitions for rhinosinusitis in children.

More emphasis is placed on the diagnosis and treatment of acute rhinosinusitis.

Throughout the document the terms chronic rhinosinusitis without nasal polyps and chronic rhinosinusitis with nasal polyps are used to further point out differences in pathophysiology and treatment of these two entities.

There are extensive chapters on epidemiology and predisposing factors, inflammatory mechanisms, (differential) diagnosis of facial pain, genetics, cystic fibrosis, aspirin exacerbated respiratory disease, immunodeficiencies, allergic fungal rhinosinusitis and the relationship between upper and lower airways.

The chapters on paediatric acute and chronic rhinosinusitis are totally rewritten.

Last but not least all available evidence for management of acute rhinosinusitis and chronic rhinosinusitis with or without nasal polyps in adults and children is analyzed and presented and management schemes based on the evidence are proposed.

IDSA clinical practice guideline for acute bacterial rhinosinusitis in children and adults.

Evidence-based guidelines for the diagnosis and initial management of suspected acute bacterial rhinosinusitis in adults and children were prepared by a multidisciplinary expert panel of the Infectious Diseases Society of America comprising clinicians and investigators representing internal medicine, pediatrics, emergency medicine, otolaryngology, public health, epidemiology, and adult and pediatric infectious disease specialties.

Recommendations for diagnosis, laboratory investigation, and empiric antimicrobial and adjunctive therapy were developed.

This article is the result of consensus reached by a working group of clinical experts in paediatric allergology as well as representatives from an ethical committee and the European Medicine Agency (EMA).

The manuscript covers clinical, scientific, regulatory and ethical perspectives on allergen-specific immunotherapy in childhood.

Unmet needs are identified.

To fill the gaps and to bridge the different points of view, recommendations are made to researchers, to scientific and patient organizations and to regulators and ethical committees.

Working together for the benefit of the community is essential.

The European Academy of Allergy and Clinical Immunology (EAACI) serves as the platform of such cooperation.
