Abstract

Food allergy (FA) is an important atopic disease although its precise burden is unclear. This systematic review aimed to provide recent, up-to-date data on the incidence, prevalence, time trends, and risk and prognostic factors for FA in Europe. We searched four electronic databases, covering studies published from 1 January 2000 to 30 September 2012. Two independent reviewers appraised the studies and qualified the risk of bias using the Critical Appraisal Skills Programme tool. Seventy-five eligible articles (comprising 56 primary studies) were included in a narrative synthesis, and 30 studies in a random-effects meta-analysis. Most of the studies were graded as at moderate risk of bias. The pooled lifetime and point prevalence of self-reported FA were 17.3% (95% CI: 17.0-17.6) and 5.9% (95% CI: 5.7-6.1), respectively. The point prevalence of sensitization to ≥1 food as assessed by specific IgE was 10.1% (95% CI: 9.4-10.8) and skin prick test 2.7% (95% CI: 2.4-3.0), food challenge positivity 0.9% (95% CI: 0.8-1.1). While the incidence of FA appeared stable over time, there was some evidence that the prevalence may be increasing. There were no consistent risk or prognostic factors for the development or resolution of FA identified, but sex, age, country of residence, familial atopic history, and the presence of other allergic diseases seem to be important. Food allergy is a significant clinical problem in Europe. The evidence base in this area would benefit from additional studies using standardized, rigorous methodology; data are particularly required from Eastern and Southern Europe.

Ο επιπολασμός της τροφικής αλλεργίας παραμένει σταθερός τα τελευταία χρόνια αν και υπάρχουν ενδείξεις για μία αυξητική τάση σύμφωνα με τα δεδομένα που επεξεργάστηκε η ομάδα για την μελέτη της τροφικής αλλεργίας της Ευρωπαϊκής Ακαδημίας Αλλεργιολογίας. Η συχνότητα της αναφερόμενης τροφικής αλλεργίας αγγίζει το 17% μειώνεται όμως σημαντικά σε ένα ποσοστό κάτω του 1% όταν για την επιβεβαίωσή της διενεργούνται τροφικές προκλήσεις. Επιπλέον η συχνότητα της τροφικής αλλεργίας είναι υψηλότερη στα παιδιά συγκριτικά με τους ενηλίκους ενώ παρατηρείται σημαντική γεωγραφική ετερογένεια μεταξύ βορρά και νότου: στις χώρες της Νότιας Ευρώπης η τροφική αλλεργία είναι λιγότερο συχνή.

Abstract

BACKGROUND:

Food allergy is a common cause of anaphylaxis, but the incidence of fatal food anaphylaxis is not known. The aim of this study was to estimate the incidence of fatal food anaphylaxis for people with food allergy, and relate this to other mortality risks in the general population.

METHODS:

We undertook a systematic review and meta-analysis, using the generic inverse variance method. Two authors selected studies by consensus, independently extracted data, and assessed the quality of included studies using the Newcastle-Ottawa assessment scale. We searched Medline, Embase, PsychInfo, CINAHL, Web of Science, LILACS or AMED, between January 1946 and September 2012, and recent conference abstracts. We included registries, databases or cohort studies which described the number of fatal food anaphylaxis cases in a defined population and time period, and applied an assumed population prevalence rate of food allergy.

RESULTS:

We included data from 13 studies describing 240 fatal food anaphylaxis episodes over an estimated 165 million food-allergic person years. Study quality was mixed, and there was high heterogeneity between study results, possibly due to variation in food allergy prevalence and data collection methods. In food allergic people, fatal food anaphylaxis has an incidence rate of 1.81 per million person years (95%CI 0.94, 3.45; range 0.63, 6.68). In sensitivity analysis with different estimated food allergy prevalence, the incidence varied from 1.35 to 2.71 per million person years. At age 0-19 the incidence rate is 3.25 (1.73, 6.10; range 0.94, 15.75; sensitivity analysis 1.18 to 6.13). The incidence of fatal food anaphylaxis in food allergic people is lower than accidental death in the general European population.

CONCLUSION:

Fatal food anaphylaxis for a food allergic person is rarer than accidental death in the general population.

Η συχνότητα θανάτου από αναφυλαξία λόγω τροφικής αλλεργίας είναι χαμηλός και δεν φαίνεται να υπερβαίνει τον κίνδυνο από θάνατο λόγω ατυχών συμβάντων άλλης αιτιολογίας. Παρ΄όλ΄αυτά οι UmasuntharT  και συν υπογραμμίζουν πως η η σωστή εκπαίδευση, η αποφυγή των υπεύθυνων αλλεργιογόνων, η σωστή επισήμανση των τροφικών αλλεργιογόνων στις ετικέτες των τροφίμων, και η γνώση για την σωστή θεραπευτική αντιμετώπιση των αναφυλακτικών επεισοδίων εξακολουθούν να αποτελούν σημαντικούς άξονες για την πρόληψη και προφύλαξη από σοβαρές και απειλητικές για την ζωή συστηματικές αντιδράσεις.

Abstract

The increase in the incidence of food allergy is a growing problem for the western world. This review will focus on the findings from several macromolecular epithelial transport experiments and drug permeability studies to provide a recent comprehension of food allergen intestinal epithelial cell transport and the allergen-epithelial relationship. Specifically, this review will aim to answer whether allergens can permeate the intestinal barrier directly via intestinal epithelial cells, and whether this mode of transport affects downstream immune reactions. By improving our understanding of the interactions which take place during exposure of food allergens with the intestinal epithelium, we can begin to understand whether the epithelial barrier plays a major role in the allergic sensitization process rather than simply restricting the entry of allergens to the underlying lamina propria.

Abstract

Ιmmunoglobulin (Ig) E–mediated food allergy is a type 2 helper T cell (TH2)–dependent disease whose prevalence is increasing in industrialized countries as a direct consequence of reduced tolerance to food antigens. The generation of regulatory T cells (Treg) is a key component of oral tolerance, and compelling experimental evidence has demonstrated that functional allergen-specific Treg cells play a major role in healthy immune responses to allergens and clinically successful allergen-specific immunotherapy. In the particular case of IgE-mediated food allergy, further investigations are required to firmly demonstrate the role of Treg cells during desensitization, induction of tolerance, or both, and several studies have also suggested a key role for these cells in healthy responses to food allergens. Treg cells are able to suppress the sensitization and effector phases of allergic reactions via several mechanisms of action based on multiple soluble and surface-binding molecules. Our knowledge of the mechanisms governing the generation of food allergen–specific Treg cells in the gastrointestinal mucosa, including the specific dendritic cell subsets involved in such processes, has increased significantly over the last decade. The identification of alternative tissues where oral tolerance to food allergens might occur in vivo is crucial, not only for a better understanding of the pathophysiology of food allergy, but also for the development of alternative therapeutic interventions. Recent findings demonstrate that oral tolerance can be induced in the tonsils through generation and maintenance of functional allergen-specific Treg cells. Further investigation in this area could pave the way for novel treatments of food allergy and other immune tolerance–related diseases.
